Please complete all questions, if not applicable please type N/A.WARNING: NCAA rules prohibit coaches from contacting freshmen and sophomores in high school. Any prospect of any age is permitted to complete and submit this questionnaire. Coaches may send written correspondence, including electronic mail, to juniors and seniors. Only seniors may receive telephone calls from coaches. Coaches may receive telephone calls from all prospects, regardless of age, but may only call or return calls to high school senior or post-graduate prospects.
